PHYSICALS
Please print and take this form to your athlete's annual physical. Physicals must be uploaded to Family ID before an athlete will be allowed to participate.

FAMILY ID
In order to play sports at the Porter Ridge Schools, all athletes MUST register through FAMILY ID.
